• Official Birth and Death Records (with Raised Seal) are available during normal business hours .
  • Birth Certificates : $15.00 each
  • Death Certificates: $14.00 for first, $11.00 for each additional (same person)
  • Picture Identification is required (ex. Driver's License).
  • Certificates for MISSOURI are printed on site. 

Safe Crib Program -Apply with Lisa Miller

  • Client is a Missouri resident who is 34 weeks gestation to 6 months postpartum
  • Client is eligible for WIC, Medicaid, or has income below 185% of the federal poverty level.
  • Family has no crib and no other resources for obtaining one
  • Family agrees to participate in two safe sleep educational sessions, with at least one session provided in the home
